JOB REQUIREMENTS: Manitowoc County Human Services Department is seeking a Clinical Case Manager. This position provides and facilitate services for individuals with a mental illness with the goal of avoiding institutionalization and enabling the individuals to live in the community in the most productive and least restrictive manner possible. This position: Provides group and individual counseling to mentally ill clients. Provides case management, direct services and coordination of services with other providers. Assists clients with improving daily living skills.
